On behalf of the Applicant Customer I acknowledge, accept and agree that:

• If Easy Fax accepts this application, the above services will be provided in accordance with Easy Fax’s Standard Terms and Conditions located at Terms of Service
• I appoint Easy Fax as my agent to act with my full authority in relation to this application
• Invoices will be sent electronically via email to your nominated email address
• Easy Fax will attempt to deliver services by the date requested, however some service delivery may not be completed by my requested date
• Information collected by Easy Fax will be handled in accordance with its Privacy Policy at Privacy Policy
• I consent to the email address being provided to receive invoices, delivery notifications, service information and marketing material from EasyFax, its related companies & partners unless I choose to opt-out
• I acknowledge and agree to pay for all services listed in this application (Pro-Rated) one month in advance which may include a part month charge if the services become active prior to the Pro-Rated charge being paid
• I acknowledge and accept that it is my responsibility to cancel my previous services which I no longer require or need from my losing provider
• Easy Fax is completely exempt from any costs/ obligations (including early termination &/or porting fees) from my losing provider
• All prices are inclusive of GST
• Total minimum amount is dependent on the plan I choose and is not limited to additional charges which can be added to my account as shown in the Critical Information Summary associated with this product which is found at Critical Information Summary
• I acknowledge that the Minimum Payable amounts shown for each service type may be subject to a bundled services offer. In the event that an eligible bundled service is cancelled, standard pricing will apply to remaining services
• Dishonored direct debit payments may incur a charge
• I agree and accept that the term of the contract begins once the service(s) becomes active
• I accept and agree to pay the charges for Credit Card or Bank Account Direct Debit payments charged by my Bank or Credit Card provider
• If I choose to cancel my service or it is disconnected for any reason within the contract term I will be charged an early termination fee of the minimum monthly amount multiplied by the remaining months in the contract term
• I agree to give 30 days’ notice to Easy Fax to disconnect, relocate or change my service which I accept & agree to pay any charges incurred
• Setup Charges assumes a standard installation and service delivery. Should additional or non-standard installation and service delivery charges apply, which may be unforeseen and may vary due to any reason, charges will be added to my account once the service(s) become active
• Relocations or any changes to services supplied by Easy Fax are subject to individual site and service qualification and subject to approval and may incur fees which I accept and agree to pay
• I accept the recurring Monthly fees, Connection fees, Setup costs, Service inclusions and Acceptable & Fair Usage terms as set out in the Critical Information Summary associated with this product which can be found at Critical Information Summary
• Acceptable and Fair Usage applies to the product relating to this application which I accept
• I accept and agree that there is no service uptime guarantee associated with the products in this application. If the service is down for any
  reason there will be no rebate I can claim. If the service is down for any reason generally it will be due to upstream carrier activates or infrastructure issues which Easy Fax cannot control
• If I am offered any credits or rebates for any reason, those credits and rebates will be applied to my Easy Fax billing account

NOTICE TO CONSUMER: You have a right to cancel this agreement within 10 business days from and including the day after you signed or received
